Any specialty electronics shop like Radio shack should sell them. I used to have one that I used to tape work calls. It had a phone cord pigtail on one end, a phone jack on the other and a microphone cord on the side that plugged into a standard 3.5mm mic jack on a tape recorder. You basically would plug in the cord to your handset jack on the phone's base, then plug the handset cord into the jack on the adapter. Only works on corded phones, although you might be able to plug it into the wall chord and get the same result with a cordless phone.
I saw your other question about using your PC to record calls, and provided you run your phone line through your modem, you can get call monitoring software, but it will only record calls made from that extension to my knowledge. It will save the call as a MP3 or other audio format on your hard disk.

you can find one on ebay, type in the search "2.5mm to 3.5mm microphone" and you should get quite a few results. Just make sure it has a built in microphone so that you can talk back to the person
